ABSTRACT

Resumen Introducción. Los cambios en el saber y en la práctica han logrado consolidar a la fisioterapia como una profesión emergente de rápido crecimiento; sin embargo, las dinámicas del mercado y los sistemas de salud han transformado las características del recurso humano, las condiciones de trabajo y los avances y tendencias de la práctica profesional. Objetivo. Caracterizar el estado actual de la práctica profesional de la fisioterapia en el marco de organización del sistema de salud colombiano, cuyas transformaciones han impactado el conjunto de prácticas profesionales del sector salud. Materiales y métodos. Se realizó una encuesta electrónica a estudiantes de Fisioterapia de la Universidad Nacional de Colombia graduados durante el período 1980-2012 que evalúa campos y áreas de desempeño, condiciones de trabajo, autonomía profesional, valoración social y perfil de morbilidad. Resultados. La fisioterapia aparece como una profesión socialmente requerida, con campos y áreas de desempeño que se proyectan más allá del sector salud. Sin embargo, son preocupantes las condiciones de deterioro del trabajo, destacando las formas de contratación, los bajos salarios y las restricciones para la práctica profesional autónoma, en particular en el campo asistencial. Conclusiones. Es necesario garantizar condiciones de trabajo que promuevan una práctica profesional idónea y que garanticen la permanencia de los profesionales, en especial en la práctica asistencial.


Abstract Introduction: Changes in knowledge and practice have consolidated physiotherapy as an emerging fast-growing profession. However, the market dynamics and the configuration of health systems have transformed the characteristics of human resources, as well as the working conditions, advances and trends of professional practice. Objective: To characterize the current state of the professional practice of physiotherapy in the organizational framework of the Colombian health system, since its transformations have impacted the professional practices of the health sector. Materials and methods: An online survey was applied to physiotherapists graduated from Universidad Nacional de Colombia between 1980 and 2012. The survey evaluates the fields and areas of performance, working conditions, professional autonomy, social assessment and morbidity profile. Results: Physiotherapy is conceived as a profession required by society, with fields and areas of performance projected beyond the health sector. However, the deteriorating work conditions are worrying, particularly recruitment, low wages and restrictions for autonomous professional practice, especially those related to healthcare services. Conclusions: It is necessary to ensure working conditions that promote a suitable professional practice and the permanence of professionals, particularly in clinical practice.

Resumen Introducción. El estudio nacional de las condiciones de salud y trabajo de los fisioterapeutas no contaba con instrumentos diseñados y validados, coherentes con el marco analítico amplio de la relación entre salud y trabajo. Objetivo. Presentar los procesos de construcción y validación del instrumento denominado evaluación de condiciones de salud y trabajo de los fisioterapeutas en Colombia. Materiales y métodos. Se utilizó el coeficiente alfa de Cronbach para medir la relación entre las respuestas a cada variable con la magnitud de cada categoría, estableciendo mediciones fijas y consistentes con un elevado nivel de correlación. El proceso de validación se realizó en 1 711 fisioterapeutas de Colombia egresados desde el año 1980 hasta 2012. Resultados. La valoración de coherencia interna a través del alfa de Cronbach encontró cinco categorías del instrumento altamente coherentes, tres con valores mayores de 0.8: condiciones peligrosas del trabajo, organización de las tareas, satisfacción con el empleo, y dos con valoraciones superiores a 0.9: eventos en salud-enfermedad e identidad y autonomía. Discusión. El diseño y construcción del instrumento de valoración de las condiciones de salud y trabajo de los fisioterapeutas nacen de la necesidad del colectivo de fisioterapeutas para establecer líneas de base amplias sobre la relación entre el trabajo y la salud, y entender las condiciones de trabajo como elemento estructurante de la salud y enfermedad.


Abstract Introduction: The national survey on health and work conditions of physiotherapists did not have designed and validated instruments that considered the broad analytical framework of the relationship between health and work. Objective: To present construction and validation processes of the instrument known as health conditions and physical therapists work in Colombia. Materials and methods: Cronbach's alpha coefficient was used to measure the relationship between responses to each variable and the magnitude of each category, establishing fixed and consistent measurements with a high level of correlation. The validation process was carried out with 1 711 Colombian physiotherapists, graduated between 1980 and 2012. Results: The assessment of internal consistency through Cronbach's found five highly coherent categories, three values greater than 0.8 (hazardous working conditions, organization of work, job satisfaction), and two above 0.9 (events in health and disease and identity and autonomy). Discussion: The design and construction of the instrument for assessing health and work conditions of physiotherapists are generated based on the needs of physiotherapists to establish broad baselines regarding the relationship between work and health, and to understand working conditions as a structuring element of health and disease.

Resumen Introducción. La rehabilitación integral es un concepto de difícil consenso, dado que es resultado de una evolución histórica. Objetivo. Presentar los hallazgos del estudio "Acceso a los servicios de rehabilitación integral en Colombia: una aproximación desde los conceptos y las prácticas de distintos actores sociales" y describir las comprensiones de la rehabilitación integral de parte de diversos actores desde una perspectiva comprensiva de la discapacidad. Materiales y métodos. Se realizó un estudio descriptivo-exploratorio, cualitativo y de múltiples fuentes de evidencia para identificar las percepciones de actores responsables de formular e implementar políticas públicas (FP), académicos (A), profesionales (P), representantes de organizaciones sociales (OS), gestores de servicios (GS), gestores y profesionales de servicios de educación inclusiva (EI), personas con discapacidad (PCD) y cuidadores (C), involucrados en la rehabilitación integral en una ciudad colombiana. Se realizaron entrevistas a 18 actores y se desarrollaron seis grupos focales. Para el análisis de datos, se usó el Atlas ti(r), el acuerdo entre jueces y el análisis de las narrativas. Resultados. La rehabilitación integral es percibida de distintas maneras por los actores, entre las que priman tres concepciones de ella: como resultado de un proceso de rehabilitación funcional, como derecho y como garante de inclusión social. Conclusiones. Mientras exista diversidad y dispersión acerca de la rehabilitación integral, será difícil concebir, implementar, evaluar y participar activamente en dicho proceso. En consecuencia, las barreras de acceso a ella se incrementarán y estará lejos de entenderse como un derecho que se ejerce desde una visión integral de ser humano. Dicho esto, se enuncian implicaciones para la academia, los prestadores de servicios y las políticas públicas.


Abstract Introduction: Comprehensive rehabilitation is a difficult concept since it is the result of historical evolution. Objective: To present the findings of the study "Access to comprehensive rehabilitation services in Colombia: an approach from the concepts and practices of different social actors", and to describe the perception of comprehensive rehabilitation based on the opinions of different actors from a comprehensive perspective of disability. Material and methods: A descriptive and exploratory, qualitative study was conducted, taking into account multiple sources of evidence to identify the perceptions of actors responsible for formulating and implementing public policies (PP), academicians (A), professionals (P), representatives of social organizations (SO), service managers (SM), service managers and professionals in inclusive education (IE), persons with disabilities (PWD) and caregivers (C), involved in comprehensive rehabilitation in a Colombian city. 18 interviews with actors were conducted; six focus groups were developed. For data analysis, the Atlas ti(r) software, the agreement between judges and the analysis of narratives were considered. Results: Comprehensive rehabilitation is perceived differently by the actors, and three conceptions are relevant: it is a result of a functional rehabilitation process; it is a right, and it is also a guarantor of social inclusion. Conclusions: While there is diversity and dispersion in opinions on comprehensive rehabilitation, it will be difficult to conceive, implement, evaluate and actively participate in this process. Consequently, access barriers will increase and it will be far from understood as a right exercised from an integral vision of the human being. With this in mind, implications for academia, service providers and public policies are set.

ABSTRACT

Introducción: en Colombia, la reforma del sistema de salud generó inequidades en el acceso a los servicios, entre ellos la rehabilitación; escasa identificación de necesidades de la población y baja respuesta intersectorial a sus problemáticas. Metodología: estudio exploratorio, descriptivo-interpretativo, con enfoque cualitativo. Se desarrollaron dieciocho entrevistas individuales semiestructuradas y seis grupos focales con gestores, prestadores de servicios de rehabilitación, representantes de organizaciones sociales, académicos, cuidadores y pacientes. Resultados: se evidenció la ausencia de un enfoque de derechos, la focalización y el modelo de aseguramiento en salud como lógicas dominantes en la formulación de políticas. Existe una mirada reduccionista de la rehabilitación y modelos de organización de servicios con baja respuesta a las necesidades de la población con discapacidad. Conclusión: se plantea la necesidad de reconceptualizar el acceso a los servicios, reivindicar la rehabilitación como derecho y rediseñar modelos de organización de servicios.


Introduction: The health sector reform in Colombia, generated inequities in access to services, including rehabilitation; poor identification of population needs and low sectorial response to their problems. Metodology: An exploratory, descriptive-interpretative qualitative study was carried out. Individual semi-structured interviews and focus group were conducted to policy makers, providers, rehabilitation professionals, professors, social organizations, persons with disabilities and caregivers. Results: The absence of a rights approach, targeting and the model of health insurance, as dominant logics policy stands. There is a reductionist view of rehabilitation and services organization models with low response to the needs of people with disabilities. Conclusion: The need to reconceptualize access to services arises; rehabilitation is claimed as a right that demands redesign of services and organization models.


Introdução: Na Colômbia, a reforma do sistema de saúde gerou iniquidades no acesso aos serviços, entre eles a reabilitação; escassa identificação de necessidades da população e baixa resposta intersetorial a suas problemáticas. Metodologia: estudo exploratório, descritivo-interpretativo, com enfoque qualitativo. Desenvolveram-se 18 entrevistas individuais semiestruturadas e 6 grupos focais com gestores, prestadores de serviços de reabilitação, representantes de organizações sociais, acadêmicos, cuidadores e pacientes. Resultados: se evidenciou a ausência de um enfoque de direitos, identificando a focalização e o modelo de asseguramento em saúde como lógicas dominantes na formulação de políticas. Existe um olhar reducionista da reabilitação e modelos de organização de serviços com baixa resposta às necessidades da população com deficiências. Conclusão: se propõe a necessidade de reconceituar o acesso aos serviços, reivindicar a reabilitação como direito e redesenhar modelos de organização de serviços.

ABSTRACT

Este estudo trata da Educação em Fisioterapia e busca reconhecer e problematizar os aspectos que estruturam a relação educação - prática profissional, para identificar elementos orientadores para uma educação que promova a construção de consciência crítica. Entende-se que a educação profissional é, em termos gerais, o processo de preparação para a prática profissional, com o qual se configura uma lógica de predomínio da organização dos serviços assistenciais no processo educativo; em consequência, cabe perguntar como tal organização influencia a formulação da Educação em Fisioterapia. Destaca-se o fato de que os processos de formulação curricular e desenho das disciplinas - tanto para a formação em disciplinas da saúde em geral, como na Fisioterapia em particular - não consideram uma aproximação explícita aos sistemas e à organização dos serviços, como aspecto estruturante da própria prática profissional ao tempo que a referência das necessidades em saúde, como orientadora na definição de conteúdos curriculares, é fraca e com frequência desvirtuada, na medida em que ela reflete os próprios limites de compreensão da realidade, que com frequência negam as críticas e inclusive o desenvolvimento de outras formas de interpretação, mantendo os marcos hegemônicos do conhecimento muitas vezes influenciados pelos interesses e lógicas dos serviços que, frequentemente, acabam modelando tais necessidades. Neste sentido, é frequente que a avaliação dos processos de educação seja desenvolvida desde uma lógica autocentrada e instrumental do próprio processo educativo, que não permite a aproximação a marcos analíticos que deem conta da relação prática profissional - educação, na qual a estrutura de organização da primeira tem um papel definitivo sobre a segunda, fazendo com que o impacto da própria educação se dê em campos limitados, no sentido de animar transformações de fundo, tanto no desenvolvimento da disciplina como na prática em si. This study addresses Education in Physiotherapy and aims to recognize and problematize the aspects that structure the education - professional practice relationship, in order to identify guiding elements for an education that promotes the development of critical consciousness. Professional education is understood as, in broad terms, the preparation process for professional practice, with which is established the logic of predominance of healthcare services organization in the educational process; consequently, one question that should be asked is how such organization affects the conception of Education in Physiotherapy. Attention is drawn to the fact that the processes of curriculum formulation and discipline design - both for the study of health disciplines in general and Physiotherapy in particular - do not take into consideration an explicit approach to systems and service organization, as a structural aspect of the professional practice itself; moreover, the approach to health needs, as a guidance in the formulation of curriculum contents, is weak and often misrepresented, since such approach reproduces the own limits of understanding reality, and frequently denies not only criticism but even the development of other forms of interpretation, maintaining the hegemonic frameworks of knowledge often influenced by the services interests and logics, and which finally shape the needs. In this sense, the evaluation of the education processes are often developed from a self-centered and instrumental perspective of the educational process itself, which does not allow the approach to analytical frameworks that give full account of the professional practice - education relationship, in which the former organization structure plays a key role on the latter; this means that the impact of education itself takes place in limited fields, regarding background changings, both in discipline development and practice itself.
